  • Prophylactic treatment of cystine urinary calculi: Increase water intake and urine diuresis. Alkalinize urine (pH 6.5-7.0) using potassium citrate. In cases with low cystine excretion and low urolith recurrence rate, tiopronin dose may be individually decreased (<30 mg/kg) or stopped.
  • Dissolution of uroliths: Reevaluation of uroliths with ultrasound or radiography every 4th week. After urolith dissolution, give prophylactic dose. If urolith dissolution is not achieved after 2-3 months, surgery is recommended.
  • Treatment of cystine urinary calculi: Relatively high incidence of adverse effects

Resumen

Tiopronina es un agente antiurolítico especializado utilizado principalmente en medicina veterinaria para controlar y prevenir la urolitiasis por cistina (cálculos en la vejiga) en perros. La cistinuria es un defecto metabólico hereditario en la reabsorción tubular renal de ciertos aminoácidos, notablemente la cistina, la cual es altamente insoluble en orina ácida y propensa a formar cálculos.

Mecanismo de acción

Tiopronin is an active sulfhydryl (-SH) compound. In the urine, it undergoes a thiol-disulfide exchange with cystine (which is a cysteine-cysteine disulfide).

  • ​Mechanism Pathway:​ Tiopronin + Cystine → Tiopronin-Cysteine Disulfide Complex + Cysteine.
  • This newly formed complex is up to 50 times more water-soluble than cystine alone.
  • By increasing the solubility of cystine in the urine, tiopronin prevents the precipitation of cystine crystals and facilitates the gradual dissolution of existing cystine calculi.

Precauciones

​Hematologic Toxicity:​ Can cause severe blood dyscrasias. Use with extreme caution in patients with pre-existing hematologic abnormalities.

​Immune-Mediated Reactions:​ Associated with Coombs'-positive regenerative spherocyte anemia in dogs. If this occurs, discontinue immediately and initiate appropriate treatment (corticosteroids, blood component therapy).

​Organ Impairment:​ Use cautiously in patients with impaired renal or hepatic function.

​Pregnancy/Nursing:​ FDA Category C. Teratogenic effects (skeletal defects, cleft palates) and decreased fetus viability have been noted with related drugs (penicillamine) or at high doses. Not recommended for use in nursing animals as it may be excreted in milk.

Monitorización

  • Physical examination (1, 3, 6, & 12 months after start, then twice a year)
  • Ultrasonography/radiography of the urinary tract
  • Urinalyses (specific gravity, protein, pH, sediment, and cyanide nitroprusside reaction) using AM samples
  • CBC (with platelets)
  • Liver enzymes (alkaline phosphatase, ALT)
  • Quantitative measurements of urinary cystine excretion related to urinary creatinine excretion (AM samples, before start and once a year during treatment)

Rapid onset of action.

Eliminación

Primarily via renal routes. In humans, up to 48% of a dose is found in the urine within 4 hours of dosing. Relatively short duration of action (effect in humans disappears in about 10 hours).

Sobredosis

There is little information available regarding acute toxicity or overdosage of tiopronin in veterinary species. It is suggested to contact an animal poison control center for further information and guidance in the event of an overdose situation. Treatment would likely be supportive and symptomatic.
