概要
ドラメクチンは、アベルメクチン系抗寄生虫薬に属するマクロライド系の薬剤です。
- 産業動物への使用: 牛および豚の消化管内線虫、肺虫、眼虫、ウシバエ幼虫、シラミ、ヒゼンダニの治療薬として、注射剤および外用ポアオン剤がFDAに承認されています。
- 伴侶動物への使用: 犬および猫において、主に全身性毛包虫症（ニキビダニ症）や犬回虫症の治療に適用外使用（適応外処方）されます。
臨床上のポイント: 非常に有効ですが、小動物への使用は完全に適用外であり、特にMDR1（ABCB1）遺伝子変異を持つ牧羊犬種では重篤な神経毒性のリスクを伴います。
作用機序
Doramectin exerts its antiparasitic effect by binding to glutamate-gated chloride channels found in invertebrate nerve and muscle cells.
- Mechanism: Binding → increases membrane permeability to chloride ions → cellular hyperpolarization → flaccid paralysis and death of the parasite.
- It also enhances the release of gamma-aminobutyric acid (GABA) at presynaptic neurons, further inhibiting nerve transmission.
- Mammalian Safety: Mammals lack glutamate-gated chloride channels, and avermectins typically do not cross the intact mammalian blood-brain barrier (where GABA receptors reside) due to the P-glycoprotein efflux pump. Toxicity occurs when this pump is deficient (e.g., MDR1 mutation) or overwhelmed by massive overdoses.

- Dogs with MDR1-allele mutations (relative/absolute depending on dose)
- Female dairy cattle 20 months of age or older
- Calves to be processed for veal

- Pupil dilation (mydriasis) in dogs
- Lethargy
- Blindness
- Coma (especially in MDR1-mutant dogs)
注意事項
The manufacturer warns against use in unapproved species due to potential severe adverse reactions (including fatalities in dogs). Extreme caution or avoidance is recommended in herding breeds susceptible to the MDR1 (ABCB1) mutation (e.g., Collies, Australian Shepherds, Shelties, Long-haired Whippets) due to a significantly higher risk for severe neurotoxicity.

Bioavailability is, for practical purposes, equal with SC and IM injections in cattle. After SC injection, time to peak plasma concentration in cattle is about 5 days.
分布
Does not readily cross the blood-brain barrier where mammalian GABA receptors occur.
過剰投与
In field trials, no toxic signs were seen in cattle given up to 25X the recommended dose. In breeding animals (bulls, and cows in early and late pregnancy), a dose 3 times the recommended dose had no effect on breeding performance.

The injectable solution should be stored below 86°F (30°C). The topical pour-on solution should be stored below 30°C (86°F) and protected from light.
飼主向け情報
- 投与の指示: 獣医師の指示に厳密に従ってください。犬や猫に適用外使用する場合は、獣医師から説明されたリスクを十分に理解してください。
- 毒性の警告: 犬や猫において、瞳孔散大、極度の無気力、ふらつき、失明、昏睡などの毒性兆候がないか注意深く観察してください。これらの症状が現れた場合は、直ちに獣医師に連絡してください。
- 休薬期間（産業動物）:
- 牛: 治療後35〜45日間は食用としてと畜しないでください（特定の製品ラベルを確認してください）。
- 豚: 治療後24日間はと畜しないでください。
- 制限事項: 生後20ヶ月以上の搾乳牛、または仔牛肉として加工される子牛には使用しないでください。反芻前の子牛における休薬期間は確立されていません。
