エドロホニウム
エドロホニウムは短時間作用型の可逆的アセチルコリンエステラーゼ阻害薬であり、主に獣医療において**重症筋無力症**の診断（テンシロンテスト）に使用されます。また、筋無力症クリーゼ（治療不足）とコリン作動性クリーゼ（過剰治療）の鑑別、迷走神経作用を介した心房頻拍の治療、非脱分極性神経筋遮断薬の拮抗にも利用されます。 > **臨床上の警告:** 重度の徐脈や心停止のリスクがあるため、エドロホニウム試験を実施する際は常にアトロピンをシリンジに準備しておく必要があります。
作用機序: Edrophonium acts as a reversible and short-acting competitive inhibitor of the enzyme **acetylcholinesterase** at the neuromuscular junction. **Acetylcholine** release → binds to receptors → normally broken down by acetylcholinesterase. Edrophonium blocks this enzyme → **acetylcholine** accumulates → prolonged and increased stimulation of nicotinic and muscarinic receptors, temporarily improving muscle strength in myasthenia gravis.

過量投与
Overdosage may lead to **muscle fasciculations** and **paralysis**. Severe bradyarrhythmias, even asystole, may occur if edrophonium is used to antagonize neuromuscular block without the co-injection of atropine. > **Treatment:** Respiration should be supported and **atropine** administered IV to counteract muscarinic effects.
