概要

エノキサパリンは​低分子量ヘパリン (LMWH)​であり、主に深部静脈血栓症 (DVT)、肺塞栓症 (PE)、猫の大動脈血栓塞栓症 (FATE) などの血栓塞栓性疾患の予防および治療に使用されます。

標準的な未分画ヘパリン (UFH) と比較して、エノキサパリンなどのLMWHはより予測可能な薬物動態プロファイルを提供し、トロンビンよりも第Xa因子を優先的に阻害します。この選択的作用により、標準的な凝固検査（aPTTなど）は有意に延長されず、出血のリスクは低くなりますが、モニタリングには専門的な検査が必要となります。

​臨床上のポイント：​

  • LMWHはヒトにおいてUFHよりも理論的に安全で予測可能ですが、犬や猫ではクリアランスが速いため、頻繁な皮下投与（通常6時間ごと）が必要になります。
  • この頻繁な投与スケジュールと高額な薬剤費により、特に大型犬や馬における獣医療での長期管理は困難になる場合があります。
  • 猫における最近の研究では、抗Xa活性レベルが実際の抗血栓効果と完全には相関しない可能性が示唆されており、治療のモニタリングが複雑になっています。

作用機序

Enoxaparin exerts its anticoagulant effect by binding to and accelerating the activity of ​antithrombin III (ATIII)​.

  • The enoxaparin-ATIII complex preferentially inhibits Factor Xa (→ blocking the conversion of prothrombin to thrombin in the coagulation cascade).
  • Because of its smaller molecular size compared to unfractionated heparin, enoxaparin has minimal impact on ​thrombin (Factor IIa)​.
  • This selective anti-Xa activity prevents clot formation and extension without significantly altering standard clotting times (TT or aPTT).

  • Hypersensitivity to enoxaparin, other LMWHs, heparin, or porcine products
  • Intramuscular (IM) or Intravenous (IV) administration (must be given SC only)

  • Nausea
  • Fever
  • Asymptomatic, reversible increases in AST or ALT

注意事項

Use cautiously in patients with significant renal dysfunction, as drug accumulation could result.

​Important:​ Do NOT administer via IM or IV routes; enoxaparin must be given via deep subcutaneous injection only. Enoxaparin cannot be used interchangeably with other LMWHs (e.g., dalteparin) or unfractionated heparin sodium because the dosages and anti-Xa activities differ for each product.

  • CBC (with platelet count); baseline and ongoing during therapy
  • Urinalysis
  • Stool occult blood test
  • Factor Xa activity (useful if bleeding occurs or patient has renal dysfunction)
  • Note: Routine coagulation tests (aPTT, PT) are usually insensitive measures of activity and usually not warranted

After SC injection in humans, absorbed rapidly with a bioavailability of about 92%; peak plasma levels (activity) occur in 3-5 hours.

分布

Anti-factor Xa activity persists for up to 24 hours in humans, but duration is significantly shorter in dogs and cats.

消失

Excreted in the urine as both unchanged drug and metabolites.

過剰投与

Overdosage may lead to hemorrhagic complications.

If treatment is necessary, protamine sulfate may be administered via slow IV as an antidote.

  • ​Dose:​ 1 mg of protamine sulfate can inhibit the effects of 1 mg of enoxaparin.
