概要
フルチカゾンは強力な合成糖質コルチコイドであり、主に吸入により猫の喘息や犬の慢性気管支炎などの慢性気道炎症を管理するために獣医療で使用されます。
臨床のポイント： 吸入投与により薬剤が直接気道粘膜に届くため、全身への吸収が最小限に抑えられ、経口コルチコステロイドに特有の副作用が大幅に軽減されます。
作用機序
Binds to specific glucocorticoid nuclear receptors → alters gene transcription → suppresses multiple inflammatory pathways, decreasing the production of inflammatory cytokines and inhibiting inflammatory cells such as eosinophils and macrophages.
安全性・警告
禁忌
- Status asthmaticus (not for acute bronchospasm relief)
- Untreated respiratory tract infections
- Known hypersensitivity to fluticasone
有害事象
- Pharyngeal candidiasis (rare)
- Mild systemic adrenal suppression (at very high doses)
- Coughing or gagging during administration
注意事項
Must be used with a veterinary-specific spacer device (e.g., AeroKat, AeroDawg) and a well-fitting face mask for effective delivery to the lower airways. This is a maintenance medication and is not effective as a rescue inhaler for acute respiratory distress. It may take several days to weeks to reach full clinical effect.

- Clinical signs of respiratory disease (coughing frequency, wheezing, respiratory rate and effort)
- Signs of systemic corticosteroid excess (PU/PD, polyphagia) if on high doses

吸収
Minimal systemic absorption from the lungs. Any swallowed fraction has very low oral bioavailability due to poor aqueous solubility and extensive first-pass metabolism.

Locally distributed in the respiratory tract.
代謝
Extensive first-pass metabolism in the liver (via CYP3A4) to an inactive carboxylic acid metabolite if swallowed.
消失
Primarily excreted in feces.
過剰投与
Systemic toxicity is highly unlikely with acute overdose due to poor oral bioavailability. Chronic massive overdose may lead to iatrogenic hyperadrenocorticism (Cushing's syndrome) or hypothalamic-pituitary-adrenal (HPA) axis suppression.

保管・安定性
Store at room temperature (below 30°C). Protect from frost and direct sunlight. Do not puncture, break, or burn the pressurized aerosol canister, even when empty.

フルチカゾンは、ペットの肺の慢性的な炎症を抑えるための予防用吸入薬です。
重要： 喘息の緊急発作や急性の呼吸困難には使用できません。
- 薬が確実に肺に届くよう、必ず処方されたスペーサーとマスクを使用してください。
- 吸入器を押した後、マスク越しにペットに7〜10回通常の呼吸をさせてください。
- 皮膚の炎症や局所の真菌感染を防ぐため、使用後は濡れた布でペットの顔を拭いてください。
