概要

ゲムフィブロジルは、フィブラート系に属する経口​抗高脂血症薬​です。

​臨床のポイント:​ 主に人間医療で使用されますが、獣医療では、食事療法（超低脂肪食）が失敗した場合の犬や猫の重度な高トリグリセリド血症（通常500 mg/dL以上）に対して適応外使用されます。ミニチュア・シュナウザーは原発性高脂血症になりやすい代表的な犬種であり、この治療が必要になることがあります。

主な特徴:

  • 厳格な脂肪制限食と併用する​補助療法​として使用されます。
  • インフルエンザ関連の死亡率を低下させる可能性があります（マウスモデルで確認）。
  • 獣医療における臨床経験は非常に限られており、有効性や安全性は完全には確立されていません。

作用機序

Gemfibrozil acts primarily as an agonist for the ​Peroxisome Proliferator-Activated Receptor-alpha (PPAR-α)​.

  • PPAR-α activation → alters transcription of genes involved in lipid metabolism.
  • Increases ​lipoprotein lipase (LPL)​ activity → enhances clearance of triglyceride-rich lipoproteins.
  • Inhibits lipolysis in adipose tissue → reduces hepatic uptake of plasma free fatty acids → decreases hepatic production of triglycerides.
  • Inhibits the synthesis of apolipoprotein B (the primary carrier protein for VLDL) → reduces VLDL production and the incorporation of long-chain fatty acids into triglycerides.

Use with extreme caution in dogs or cats as very limited safety data is available for this medication.

​Pregnancy Warning:​ In humans, it is FDA Category C. Animal studies (rats/rabbits) have shown decreased fertility rates, decreased litter sizes, and skeletal abnormalities at doses near the human dose. Safe use during nursing cannot be assured.

In humans, rapidly and completely absorbed from the GI tract. Rate and extent of absorption are greatest when administered 30 minutes before a meal. (No veterinary data available).

分布

Highly bound to plasma protein. Highest concentrations are found in the liver and kidneys.

代謝

Hepatic metabolism; 4 major metabolites are formed in humans.

消失

Primarily excreted in the urine.

過剰投与

Limited veterinary information is available. The reported LD50 (oral) in rats is 1414 mg/kg.

  • ​Treatment:​ Consider gut-emptying protocols for recent large oral ingestions and provide supportive care as required.
  • ​Monitoring:​ Monitor for dehydration and electrolyte imbalance if vomiting and/or diarrhea is severe or persists. Monitor liver function tests.
