グリコピロニウム / グリコピロレート
**グリコピロニウム**（別名**グリコピロレート**）は、強力な抗コリン薬および抗唾液分泌薬です。主に獣医麻酔において、口腔および気管支の分泌物を減少させ、迷走神経遠心性活動を抑制し、強力なオピオイド薬の投与によって引き起こされる徐脈を管理するために使用されます。また、神経筋遮断の拮抗時に長時間作用型抗コリンエステラーゼ薬（ネオスチグミンなど）と併用されます。 > **臨床上の警告：** 麻酔前の前投薬としてのグリコピロニウムのルーチン投与は、もはや推奨されていません。分泌物の水分含有量を減少させるため、気道分泌物がより粘稠になり、排出が困難になるためです。
作用機序: Competitively blocks the action of **acetylcholine** at **muscarinic receptors** at the terminal ends of the parasympathetic nervous system → reverses parasympathetic effects (e.g., increases heart rate, reduces salivation). **Clinical Pearl:** Because of its quaternary ammonium structure, glycopyrronium cannot cross the blood-brain barrier. Therefore, it is devoid of central nervous system effects and does not cause central mydriasis, unlike atropine. It also has a longer duration of action compared to atropine.

過量投与
Tachycardias following an overdose of glycopyrronium are usually transient and do not require management. If severe ventricular arrhythmias occur, they may be treated with **lidocaine**.
