概要
ヒプロメロースは、三層構造の涙液膜の水層成分を補うために使用されるセルロースベースの水性人工涙液です。涙液模倣薬として働き、眼に潤いを与え、乾燥を和らげます。
臨床のポイント： 動物の乾性角結膜炎（KCS、ドライアイ）の管理に頻繁に使用され、シクロスポリンやタクロリムスなどの局所用涙液分泌促進薬と併用されることがよくあります。水性の代替品であるため、粘度の高い軟膏やゲルと比較して頻繁な点眼が必要です。
作用機序
Hypromellose is a hydrophilic polymer that acts as a lacrimomimetic. It increases the viscosity of tears and prolongs corneal contact time.
Cellulose-based polymer → binds to existing tears → stabilizes the aqueous layer of the trilaminar tear film → prevents rapid tear evaporation and provides mechanical lubrication to the cornea and conjunctiva.
安全性・警告
禁忌
- Hypersensitivity to hypromellose or any component of the formulation (such as preservatives like benzalkonium chloride in multi-dose bottles)
有害事象
- Transient blurred vision immediately after application
- Mild ocular irritation or stinging (rare)
- Crusting on the eyelids from dried polymer
注意事項
Contamination Risk: Do not touch the dropper tip to any surface, including the eye or eyelid, to avoid bacterial contamination of the bottle. Timing: If using multiple topical ophthalmic medications, administer hypromellose last, or wait at least 5-10 minutes between applications to prevent washout of the preceding drug. Preservatives: Multi-dose bottles often contain preservatives (e.g., benzalkonium chloride) which can be epitheliotoxic with very frequent use (e.g., >4-6 times daily). Consider preservative-free single-use vials for severe dry eye requiring highly frequent application.

May wash out other eye drops if administered simultaneously. Hypromellose can also act as a barrier if applied before other drugs.

- Resolution of clinical signs of dry eye (e.g., redness, mucoid discharge, blepharospasm)
- Schirmer Tear Test (STT) if managing KCS
- Corneal staining (fluorescein) to monitor for corneal ulceration secondary to dry eye

過剰投与
Overdosage is highly unlikely due to the topical route of administration and lack of systemic absorption. Excess drops will simply overflow from the eye.

ヒプロメロースは、ペットの目を潤して快適に保つために使用される人工涙液です。
- 点眼する前に手を洗ってください。
- 汚染を防ぐため、ボトルの先端がペットの目、まぶた、または飼い主の指に触れないようにしてください。
- ペットが他の点眼薬を使用している場合は、異なる点眼薬を使用する間に少なくとも5〜10分間隔を空けてください。通常、人工涙液は最後に点眼します。
- 指示通りに保管し、液が濁ったり変色したりした場合は廃棄してください。
- 複数回使用できるボトルは、通常、開封後28日で廃棄する必要があります。
