概要
局所投与（吸入）されるイプラトロピウム臭化物は、馬の回帰性気道閉塞（RAO）や小動物の慢性呼吸器疾患など、気管支痙攣状態の補助治療に使用される抗ムスカリン薬です。
臨床のポイント:
- イプラトロピウムはアトロピンの第四級アンモニウム誘導体です。極性が高いため、全身循環への吸収が少なく、血液脳関門を容易に通過しないため、全身性の抗コリン性副作用のリスクが大幅に軽減されます。
- 作用時間は比較的短く、頻繁な投与が必要になる場合があります。
- 小動物における正式な薬物動態データは限られていますが、補助的な気管支拡張薬として一般的に安全かつ有効であると考えられています。
作用機序
Ipratropium acts as a competitive antagonist of acetylcholine at muscarinic receptors in the bronchial smooth muscle.
- Mechanism: Antagonizes acetylcholine → Prevents increases in intracellular cyclic guanosine monophosphate (cGMP) → Reduces bronchial smooth muscle constriction.
- Advantage over Atropine: Unlike atropine, ipratropium does not reduce mucociliary clearance, making it highly beneficial for patients with respiratory diseases where clearing airway secretions is critical.

保管・安定性
The solution for inhalation should be stored at room temperature and protected from light. Keep in foil pouch until time of use. The metered dose inhalers should be stored at room temperature. The solution for nebulization may be mixed with albuterol or metaproterenol if used within one hour.
