概要
ミコナゾールは広域スペクトルのイミダゾール系抗真菌薬であり、主に皮膚糸状菌症（白癬）やマラセチア皮膚炎などの真菌性皮膚および耳の感染症の局所治療に使用されます。外耳炎や膿皮症の治療において相乗効果を得るため、他の薬剤（クロルヘキシジン、コルチコステロイド、抗生物質など）と配合されることがよくあります。
臨床のポイント： 局所的には非常に有効ですが、潜在的な毒性があり、イトラコナゾールやフルコナゾールなどのより安全な全身性代替薬があるため、獣医学においてミコナゾールが全身的に使用されることはまれです。
作用機序
Miconazole works by inhibiting the cytochrome P450-dependent enzyme 14-alpha-demethylase in fungal cells.
Inhibition of 14-alpha-demethylase → depletion of ergosterol (a vital component of the fungal cell membrane) → accumulation of toxic sterol precursors → increased cell wall permeability → leakage of cellular contents and subsequent fungal cell death.
It has potent activity against Malassezia, Cryptococcus, Candida, and Coccidioides.

- Hypersensitivity to miconazole or other azole antifungals
- Ruptured tympanic membrane (for certain otic preparations)

注意事項
Normal precautions should be observed. Avoid contact with the patient's eyes. For otic preparations, ensure the tympanic membrane is intact before application.
Warning: When treating dermatophytosis in cats with miconazole shampoo, concurrent systemic therapy (e.g., itraconazole) is required for effective clearance.

Minimal systemic absorption occurs following topical application to intact skin.

Remains primarily localized in the stratum corneum of the skin.

過剰投与
Overdose is highly unlikely with topical administration. Accidental ingestion of shampoos or creams may cause mild gastrointestinal upset (hypersalivation, vomiting, diarrhea). Treatment is symptomatic and supportive.

ミコナゾールは、皮膚や耳の感染症の治療に使用される局所抗真菌薬です。
- 使用方法： 獣医師の指示に注意深く従ってください。シャンプーの場合は、泡を皮膚に10分間放置してから十分に洗い流してください。
- 点耳薬： 獣医師の指示がない限り、投薬直後に耳の掃除をしないでください。
- 安全上の注意： 治療部位が完全に乾くまで、ペットが舐めないようにしてください。投薬後は手をよく洗ってください。
- 白癬（皮膚糸状菌症）： 白癬の治療を行っている場合、皮膚が正常に見えるようになっても、再発を防ぐために処方された期間は治療を続けてください。
