抗真菌薬（ポリエン系）
ナイスタチンは広域スペクトルの**ポリエン系抗真菌薬**であり、特に**カンジダ**（特に*C. albicans*）に対して有効です。消化管からは吸収されないため、局所/外用または消化管の真菌感染症に適しています。 > **臨床のポイント：** 全身に吸収されないため、経口製剤は消化管のカンジダ症にのみ有効であり、外用製剤は皮膚や耳のマラセジア・酵母菌感染症に使用されます。
作用機序: Nystatin binds to **ergosterol**, a major component of the fungal cell membrane. This binding → forms pores in the membrane → leads to **potassium leakage** → results in the death of the fungus.

禁忌
- Known hypersensitivity to nystatin
有害事象
- Generally well tolerated due to lack of systemic absorption
- Rarely GI upset if given orally (nausea, vomiting, diarrhea)
- Mild irritation or hypersensitivity at the site of topical application (rare)
モニタリング
- Resolution of clinical signs of fungal/yeast infection
- Skin/ear cytology to confirm clearance of Malassezia or Candida
過量投与
Overdose is highly unlikely to cause systemic toxicity due to the lack of gastrointestinal absorption. Massive oral overdose may cause mild gastrointestinal distress (vomiting, diarrhea).
