膵酵素サプリメント
**膵酵素サプリメント**は、**膵外分泌不全（EPI）**の臨床症状をコントロールするために使用される外因性補充療法です。これらのサプリメントには、**リパーゼ、プロテアーゼ、アミラーゼ**の混合物が含まれています。 > **臨床のポイント：** EPI患者では低コバラミン血症がよく見られるため、**ビタミンB12（コバラミン）**の併用療法により有効性が著しく向上することがあります。制酸剤の併用は一般的に費用対効果が低く、給与前に酵素を食事と混ぜて事前培養（プレインキュベーション）する必要はありません。新鮮な生の豚膵臓や冷凍豚膵臓も有効な代替品となり得ますが、入手が限られており、病原体摂取のリスクがあります。
作用機序: Provides exogenous replacement of pancreatic enzymes: * **Lipase** → hydrolyses dietary triglycerides into fatty acids and glycerol. * **Protease** → cleaves peptide bonds to break down dietary proteins. * **Amylase** → hydrolyses complex carbohydrates into simpler sugars. These actions restore digestive capability in the small intestine, facilitating nutrient absorption and resolving maldigestion.

- Exocrine pancreatic insufficiency (EPI) · Use manufacturer's recommendations initially. Adjust to obtain normal faeces. Reduce gradually over 7 days to find minimally effective dose. · PO · with every meal · Lifelong · Lypex capsules must be opened and contents sprinkled in with food. Follow dosing with food or water.

過量投与
High doses may cause **diarrhoea** and signs of **gastrointestinal cramping**. Reduce dose if these signs occur.
