概要
ポナズリル（Ponazuril）は、主に Sarcocystis neurona によって引き起こされる馬原虫性脊髄脳炎（EPM）の治療に使用される、FDA承認のトリアジン系抗原虫薬です。
臨床のポイント：
- トルトラズリルの活性代謝物です。
- 馬用に承認されていますが、その有効性と比較的安全なプロファイルから、犬猫、エキゾチックアニマル、反芻動物のコクシジウム症、ネオスポラ症、トキソプラズマ症の治療にオフラベルで広く使用されています。
- 血液脳関門を通過するため、中枢神経系の原虫感染症に有効です。
作用機序
Ponazuril targets the apicoplast (plastid body), an essential organelle found specifically in members of the Apicomplexa phylum (which includes Sarcocystis, Toxoplasma, Neospora, and Eimeria).
Inhibition of the apicoplast → Disruption of the organism's ability to synthesize essential fatty acids and amino acids → Protozoal death (cidal activity).
安全性・警告
禁忌
- No absolute contraindications noted, but other causes of ataxia should be ruled out before treating for EPM.

- Blisters on nose and mouth
- Rash or hives
- Diarrhea or loose feces
- Mild colic
- Seizures (rare)
- Keratoconjunctivitis sicca (KCS) in dogs (especially predisposed breeds or at high doses)
注意事項
Safety during pregnancy or in lactating mares has not been evaluated. Use with caution in dogs predisposed to Keratoconjunctivitis sicca (KCS) and monitor tear production. The commercial equine paste is highly concentrated and must be properly diluted/compounded for accurate dosing in small animals and exotics.

- Clinical efficacy and resolution of neurological or gastrointestinal signs
- Tear production (Schirmer Tear Test) in dogs, especially predisposed breeds

In horses, approximate bioavailability is 30% when administered orally in water. Bioavailability is significantly enhanced if dissolved in DMSO. In cattle, oral doses are relatively well absorbed.
分布
In horses, peak serum levels are reached in about 18 days and peak CSF levels in about 15 days. Peak CSF levels are approximately 1/20th (0.21 mcg/mL) of those found in the serum.
代謝
Ponazuril is an active metabolite (sulfone derivative) of toltrazuril.
消失
Elimination half-life from serum in horses averages about 4.5 days (108 hours).
過剰投与
In horses, daily doses of up to 30 mg/kg (6X the normal dose) primarily caused loose feces. Moderate edema of the uterine epithelium was noted on histopathology for female horses receiving the 6X dose. In dogs, overdose quantities have been associated with the development of Keratoconjunctivitis sicca (KCS).

飼主向け情報
重要： 効果を得るためには、処方された通りに全期間投与する必要があります。
- 副作用： 発疹、じんましん、口や鼻の周りの水疱、または胃腸症状（下痢など）が見られた場合は、獣医師に連絡してください。
- EPMの馬の場合： 治療費が高額になる可能性があります。また、すでに存在する神経損傷によっては、臨床的な改善がわずかであるか、全く見られない場合があります。
- 犬の場合： ドライアイの兆候（目を細める、充血、ドロドロした目やに）に注意し、獣医師に報告してください。
