スキサメトニウム
スキサメトニウム（サクシニルコリン）は、超短時間作用型の脱分極性神経筋遮断薬です。主に獣医療において、迅速な気管挿管を容易にし、手術中の骨格筋弛緩を提供するために使用されます。 > **臨床のポイント：** 脱分極性薬剤であるため、使用前に一時的な筋線維束性収縮が見られることがよくあります。**鎮痛作用や鎮静作用はない**ため、患者は完全に麻酔され、人工呼吸管理を受ける必要があります。
作用機序: Competitively binds to the **nicotinic acetylcholine receptor** at the neuromuscular junction. The persistent depolarization prevents the transmission of further action potentials → resulting in flaccid muscle relaxation.
投与経路
禁忌
- Patients with pre-existing hyperkalemia
- Glaucoma or penetrating eye injuries
- Severe liver disease
- Patients with known atypical plasma pseudocholinesterase
- Conscious patients (must be fully anesthetized)

- Muscle fasciculations
- Hyperkalemia
- Bradycardia or arrhythmias
- Malignant hyperthermia
- Increased intraocular pressure
- Increased intragastric pressure
- Prolonged apnea

- Peripheral nerve stimulation (Train-of-Four) to assess blockade depth and recovery
過量投与
Overdose results in prolonged neuromuscular blockade and extended apnea. **Treatment is strictly supportive** with continuous mechanical ventilation and maintenance of anesthesia until the drug is fully metabolized and spontaneous respiration returns. **There is no specific pharmacological reversal agent** for depolarizing neuromuscular blockers.
