タクロリムス
タクロリムス（FK 506）は強力なマクロライド系免疫抑制剤です。獣医療では主に、免疫介在性皮膚疾患（肛門周囲瘻やアトピー性皮膚炎など）の局所治療や、乾性角結膜炎（KCS）の点眼治療に使用されます。毒性の可能性があるため全身投与は一般的ではありませんが、重度の免疫介在性疾患に利用されることがあります。 **臨床のポイント：** シクロスポリンに不応性のKCS症例に対して有効な場合が多いです。
作用機序: Binds to the intracellular protein **FKBP-12** (FK506 binding protein) → forms a complex that inhibits **calcineurin** (a calcium-dependent phosphatase) → prevents dephosphorylation of NF-AT (nuclear factor of activated T-cells) → inhibits transcription of **IL-2** and other cytokines → profound **T-lymphocyte inhibition**.

禁忌
- Hypersensitivity to macrolides
- Active systemic infections
- Malignancy (for systemic use)
有害事象
- Topical: local irritation, erythema, pruritus
- Ophthalmic: mild ocular irritation
- Systemic: GI upset (vomiting, diarrhea), nephrotoxicity, neurotoxicity, secondary infections

- Ketoconazole · Inhibits CYP3A metabolism, significantly increasing systemic tacrolimus levels · major
- Erythromycin · Inhibits metabolism, increasing tacrolimus levels · major
- Grapefruit juice · Increases systemic absorption and blood levels of tacrolimus · moderate
モニタリング
- Tear production (Schirmer Tear Test) for KCS
- Resolution of skin lesions
- Whole blood trough levels (if used orally)
- Renal and hepatic function (if used orally)
過量投与
Systemic overdose can lead to severe nephrotoxicity, neurotoxicity (tremors, seizures), and profound immunosuppression. Treatment is supportive and symptomatic. No specific antidote exists.
