概要

タムスロシンは尿路に選択性を持つ​α-1アドレナリン受容体拮抗薬​です。前立腺、尿道、膀胱頸部の平滑筋を弛緩させることで、尿の排出を助けます。プラゾシンやフェノキシベンザミンよりも作用時間が長いです。

​臨床のヒント:​ 獣医療では、機能性尿閉、未去勢雄犬の良性前立腺肥大症（BPH）の管理、または尿管結石/尿路結石の排出補助として適応外使用されることがよくあります。

作用機序

Tamsulosin selectively binds to and blocks alpha-1A and alpha-1D adrenergic receptors located predominantly in the smooth muscle of the prostate, prostatic capsule, prostatic urethra, and bladder neck.

Blockade of these receptors → relaxation of smooth muscle → decreased urethral resistance → improved urine flow and relief of functional urinary obstruction.

  • Hypersensitivity to tamsulosin
  • Severe hepatic impairment
  • History of orthostatic hypotension

  • Gastrointestinal upset (vomiting, diarrhea)

注意事項

​Caution:​ May cause hypotension. Monitor blood pressure, especially when initiating therapy or combining with other hypotensive agents. Use with caution in patients with renal or hepatic impairment.

Well absorbed from the gastrointestinal tract; absorption may be affected by food.

Highly bound to plasma proteins.

代謝

Metabolized in the liver via cytochrome P450 enzymes.

消失

Excreted primarily in the urine as metabolites and some unchanged drug.

過剰投与

Overdosage may lead to severe hypotension. Treatment is primarily supportive, including intravenous fluid therapy and potentially vasopressors if hypotension is refractory. Emesis or gastric lavage may be considered if ingestion is recent.
