テルミサルタン
**テルミサルタン**はアンジオテンシンII受容体拮抗薬（ARB）であり、獣医療では主に**猫の慢性腎臓病（CKD）**に伴う蛋白尿の軽減、および猫の**全身性高血圧**の治療に使用されます。経口液剤として提供され、一般的に忍容性は良好です。 > **臨床上の警告：** 200 mmHgを超える全身性高血圧の管理におけるテルミサルタンの安全性および有効性は検討されていません。低血圧の臨床症状を示す猫、または全身麻酔を受ける猫では、血圧のモニタリングが強く推奨されます。 **臨床のポイント：** CKD（IRISステージIIa〜IV）の猫において、6ヶ月間の蛋白尿悪化を遅らせる効果は、ベナゼプリルと同等であると考えられています。
作用機序: Telmisartan is a specific **Angiotensin II receptor (type AT1) antagonist**. By blocking this receptor, it inhibits the detrimental effects of angiotensin II, including: * **Vasoconstriction** → lowers systemic blood pressure. * **Aldosterone synthesis** → reduces sodium and water retention. * **Renal, vascular, and cardiac remodelling**. In the kidneys, blocking angiotensin II reduces **glomerular capillary hypertension** and decreases protein in the glomerular filtrate, which helps prevent the triggering or potentiation of interstitial fibrosis.

- Reduction of proteinuria associated with chronic kidney disease · 1 mg/kg · PO · q24h · Authorized indication. Use 4 mg/ml solution.
- Treatment of systemic hypertension · 2 mg/kg · PO · q24h · Authorized indication. Use 10 mg/ml solution.

In healthy cats administered **5 times the recommended dose for 6 months**, observed effects included decreases in blood pressure, decreases in red blood cell (RBC) count, and increases in blood urea nitrogen (BUN).
