概要
テトラカイン（別名アメトカイン）は、主に眼表面（角膜および結膜嚢）の局所麻酔に使用される局所麻酔薬です。
臨床のポイント： プロキシメタカインなどの代替薬と比較して、点眼時の結膜刺激や痛みが強いため、小動物の臨床ではあまり使用されません。重篤な角膜毒性があるため、継続的な治療目的で処方してはなりません。
作用機序
Local anaesthetic action is dependent on the reversible blockade of the voltage-gated sodium channels → prevents the influx of sodium → inhibits the propagation of an action potential along the nerve fibre. Sensory nerve fibres are blocked before motor nerve fibres, allowing a selective sensory blockade at low doses.

- Therapeutic use (repeated administration for pain relief)
- Application prior to a Schirmer tear test

注意事項
Diagnostic Interference: Topical anaesthetics block reflex tear production and should not be applied before a Schirmer tear test.
Corneal Toxicity: All topical anaesthetics are toxic to the corneal epithelium. Repeated administration can severely delay the healing of ulcers and lead to corneal melting.

過剰投与
Overdosage in a clinical setting typically presents as local toxicity rather than systemic effects. Repeated administration leads to severe corneal epithelial toxicity, delayed healing of corneal ulcers, and potential corneal melting.
Do not use for therapeutic purposes.
