概要

ザフィルルカストは、獣医学において適応外使用される経口​ロイコトリエン受容体拮抗薬​です。理論的には、ロイコトリエンが関与する炎症性疾患（​猫の喘息​、​犬のアトピー性皮膚炎​、​炎症性腸疾患（IBD）​など）に有効である可能性がありますが、獣医臨床における有効性は期待外れであるか、限定的、あるいは十分に研究されていません。

​臨床のポイント：​ ロイコトリエンはリポキシゲナーゼ経路を介して産生される強力な炎症メディエーターです。これらを阻害することは人間の喘息には非常に有効ですが、猫や犬は他の主要な炎症経路（ヒスタミン、インターロイキン、プロスタグランジンなど）に依存している可能性があり、これがこれらの動物種におけるザフィルルカストの臨床反応が不安定でしばしば不良である理由を説明しています。

作用機序

Zafirlukast selectively and competitively inhibits leukotriene receptors, specifically targeting the LTD4 and LTE4 receptors.

  • Arachidonic acid → 5-lipoxygenase pathway → Leukotrienes (LTC4, LTD4, LTE4)
  • By blocking these receptors, zafirlukast prevents the binding of slow-reacting substance of anaphylaxis (SRS-A).
  • This inhibition reduces smooth muscle contraction (bronchoconstriction), vascular permeability, and inflammatory cell infiltration associated with allergic and asthmatic responses.

  • Hypersensitivity to zafirlukast
  • Acute bronchospasm or acute asthma attacks

  • Vomiting (dogs)
  • Headache (humans)

注意事項

Zafirlukast is not indicated for, and is ineffective for treating bronchospasm associated with acute asthma attacks. Patients with significantly decreased hepatic function may have reduced clearances (and increased plasma levels) of zafirlukast.

Rapidly absorbed after oral administration (human data). Food may impair the absorption of the drug; therefore, give on an empty stomach. Peak plasma levels occur about 3 hours after dosing.

Highly bound to plasma proteins (>99%).

Extensively metabolized.

消失

Less than 10% of a dose is excreted in the urine, the rest in the feces.

過剰投与

In dogs, doses of up to 500 mg/kg were tolerated without mortality. Overdose is generally expected to be well tolerated, but supportive care should be provided if significant gastrointestinal signs occur.

Zafirlukast tablets should be stored at room temperature and protected from light and moisture. The manufacturer states that the tablets should be dispensed only in the original, unopened container.
