개요
브린졸라미드는 주로 개와 고양이의 녹내장 관리에 단독으로 또는 다른 국소 약물과 함께 사용되는 국소 탄산탈수효소 억제제입니다. 생리적 pH가 7.5이므로 도르졸라미드보다 내약성이 더 좋은 경우가 많습니다.
임상 팁: 브린졸라미드는 정상 고양이의 안압을 낮추는 데는 효과가 없지만 녹내장이 있는 고양이의 안압(IOP)은 성공적으로 낮출 수 있습니다. 반면, 도르졸라미드는 정상 및 녹내장이 있는 개와 고양이 모두에게 효과적입니다.
작용 기전
Brinzolamide reduces intraocular pressure (IOP) by inhibiting the enzyme carbonic anhydrase within the ciliary body epithelium.
Inhibition of carbonic anhydrase → decreased formation of bicarbonate ions → reduced fluid transport → decreased rate of aqueous humour production.

금기사항
- Severe hepatic impairment
- Severe renal impairment
- Uveitis (specifically for the timolol combination, as timolol causes miosis)
- Anterior lens luxation (timolol combination)
- Pupil block (timolol combination)
부작용
- Local ocular irritation
- Keratitis
- Blepharitis
- Hypokalaemia (cats, rare systemic effect)
- Metabolic acidosis (dogs, rare systemic effect)
- Bradycardia and hypotension (if using the timolol combination product)
주의
Normal ophthalmic handling precautions should be observed.
Warning: Systemic absorption can occur. Rarely, topical carbonic anhydrase inhibitors have been reported to cause hypokalaemia in cats and metabolic acidosis in dogs.
Combination Product Warning: Products containing timolol (a beta-blocker) can cause systemic bradycardia and hypotension, and induce miosis, making them inappropriate for patients with uveitis or anterior lens luxation.

- Signs of local ocular irritation (keratitis, blepharitis)
- Serum potassium and acid-base status (if prolonged use or systemic signs occur)
- Heart rate and blood pressure (if using timolol combination)
약동학
흡수
Administered topically to the eye, but systemic absorption can occur, potentially leading to systemic side effects (e.g., metabolic acidosis, hypokalaemia).
과다투여
Excessive topical application may increase the risk of systemic absorption, potentially leading to metabolic acidosis or hypokalaemia (especially in cats). If the timolol combination is used, overdose may lead to significant bradycardia and hypotension. Treatment is supportive and symptomatic.
