개요

데페록사민은 중금속 킬레이트제로 주로 철 중독의 해독제로 사용됩니다. 빠른 정맥 투여는 심각한 저혈압 및 아나필락시스 반응을 유발할 수 있습니다.

작용 기전

Deferoxamine chelates iron by binding to free iron (Fe3+) in the blood and tissues, forming a stable, water-soluble complex called ferrioxamine.

Free Iron + Deferoxamine → Ferrioxamine complex → Excreted in the urine.

안전성·주의사항

금기사항

  • Severe renal disease

부작용

  • Pain at IM injection site
  • Anaphylactic reactions (especially with rapid IV)
  • Hypotension (if administered rapidly IV)

주의

Normal precautions should be observed. Do not administer rapidly intravenously due to the high risk of hypotension and anaphylaxis. Use slow IV infusion instead.

모니터링

  • Serum iron levels
  • Total iron-binding capacity (TIBC)
  • Blood pressure (especially during IV infusion)
  • Renal function (BUN, Creatinine)
  • Urine color

흡수

Well absorbed after IM administration.

분포

Distributes widely but does not readily cross the blood-brain barrier.

대사

Metabolized by plasma enzymes.

배설

The deferoxamine-iron complex (ferrioxamine) is excreted primarily in the urine.

과다투여

Overdosage or rapid IV administration can lead to acute hypotension, tachycardia, and shock. Treatment is supportive, including IV fluids and slowing/stopping the infusion.
