개요

에드로포늄은 속효성, 가역적 아세틸콜린에스테라아제 억제제로, 수의학에서 주로 ​중증 근무력증​ 진단(텐실론 검사)에 사용됩니다. 또한 근무력성 위기(치료 부족)와 콜린성 위기(과다 치료)를 구별하고, 미주신경 효과를 통해 심방 빈맥을 치료하며, 비탈분극성 신경근 차단제를 길항하는 데 사용됩니다.

​임상 경고:​ 심각한 서맥이나 심수축부전의 위험이 있으므로 에드로포늄 검사를 수행할 때는 항상 아트로핀을 주사기에 준비해 두어야 합니다.

작용 기전

Edrophonium acts as a reversible and short-acting competitive inhibitor of the enzyme acetylcholinesterase at the neuromuscular junction.

Acetylcholine release → binds to receptors → normally broken down by acetylcholinesterase. Edrophonium blocks this enzyme → acetylcholine accumulates → prolonged and increased stimulation of nicotinic and muscarinic receptors, temporarily improving muscle strength in myasthenia gravis.

주의

Use with caution in patients with bronchial disease (especially feline asthma), bradycardia (and other arrhythmias), hypotension, renal impairment, or epilepsy.

​Clinical Pearl:​ If edrophonium availability is limited, neostigmine may be used as an alternative diagnostic test for myasthenia gravis.

약동학

흡수

Rapid onset of action following intravenous administration.

과다투여

Overdosage may lead to muscle fasciculations and paralysis. Severe bradyarrhythmias, even asystole, may occur if edrophonium is used to antagonize neuromuscular block without the co-injection of atropine.

​Treatment:​ Respiration should be supported and atropine administered IV to counteract muscarinic effects.
