개요

메피바카인은 신경 차단, 국소 침윤 및 관절 내 진통에 사용되는 아미드형 국소 마취제입니다. 리도카인에 비해 혈관 확장 작용이 적고 조직 자극이 덜한 것으로 여겨집니다. 작용 시간은 약간 더 길며(100-120분), 효과를 연장하기 위해 에피네프린을 추가할 필요가 없습니다. 임상 경고: 정맥 내 주사는 절대 금합니다. 부주의한 정맥 내 투여는 경련 및 심정지를 유발할 수 있습니다.

작용 기전

Local anesthetic action is dependent on the reversible blockade of the voltage-gated sodium channels → prevents the influx of sodium ions → prevents the propagation of an action potential along the nerve fiber.

Sensory nerve fibers are blocked before motor nerve fibers, allowing a selective sensory blockade at low doses.

주의

Strictly avoid intravenous injection. Inadvertent IV administration may cause severe central nervous system and cardiovascular toxicity.

Clinical Pearl: Toxic doses of mepivacaine have not been definitively established in companion animals; therefore, always use the minimal effective volume and carefully calculate maximum safe doses based on general amide local anesthetic guidelines.

과다투여

Toxic doses of mepivacaine have not been established in companion animals. Treatment is supportive and symptomatic (e.g., seizure control, cardiovascular support, potentially intravenous lipid emulsion therapy).
