개요
니자티딘(Nizatidine)은 라니티딘과 구조 및 약리가 유사한 H2 수용체 길항제입니다. 시메티딘이나 파모티딘처럼 위산 억제 능력이 있지만, 수의학(특히 소동물 임상)에서는 주로 위장관 운동 촉진(prokinetic) 특성 때문에 사용됩니다.
- 위 배출 지연, 장 가성폐색, 고양이 변비 및 거대결장증 관리에 사용됩니다.
- 고양이 췌장염에서 출혈성 괴사를 예방하는 데 도움이 될 수 있습니다.
임상 요점: 더 강력한 위산 억제제(양성자 펌프 억제제 등)와 다른 위장관 운동 촉진제가 널리 보급됨에 따라 현재는 사용 빈도가 낮지만, 위산 억제와 운동 촉진 작용이 동시에 필요할 때 여전히 유용한 선택지입니다.
작용 기전
- Acid Suppression: Competitively inhibits histamine at the H2 receptors on gastric parietal cells → reduces basal and stimulated gastric acid output → decreases pepsin secretion.
- Prokinetic Action: Inhibits acetylcholinesterase → prevents breakdown of acetylcholine → increases acetylcholine concentrations at muscarinic receptors → stimulates GI motility.
- May also exert direct agonist effects on M3 muscarinic receptors.
- Increases lower esophageal sphincter (LES) pressure.

- Anemia (rare)

- Cholinergic effects at high doses
주의
Use cautiously and consider reduced dosage in patients with diminished renal function. Monitor serum ALT in high-dose, chronic therapy. Geriatric patients or those with hepatic insufficiency require caution. False positive tests for urobilinogen may occur.

흡수
Rapid and nearly complete oral absorption with minimal first-pass effect in dogs. Food enhances absorption but is not clinically important.
분포
Marginally bound to plasma proteins. Unknown if it enters the CNS. Excreted in maternal milk at 0.1% of oral dose.
대사
Metabolized in the liver to several metabolites, including at least one active metabolite.
배설
Over 50% excreted unchanged in the urine in animals with normal renal function.
과다투여
Single oral doses up to 800 mg/kg were not lethal in dogs. Overdose may cause cholinergic effects (lacrimation, salivation, emesis, miosis, diarrhea). Treat supportively and symptomatically.
