개요
텔미사르탄(Telmisartan)은 안지오텐신 II 수용체 차단제(ARB)로, 수의학에서는 주로 고양이의 만성 신장 질환(CKD)과 관련된 단백뇨를 감소시키고 고양이의 전신 고혈압을 치료하는 데 사용됩니다. 일반적으로 내약성이 좋은 경구용 액제로 제공됩니다.
임상 경고: 200 mmHg 이상의 전신 고혈압 관리에 대한 텔미사르탄의 안전성과 효능은 조사되지 않았습니다. 저혈압과 관련된 임상 증상을 보이거나 전신 마취를 받는 고양이의 경우 혈압 모니터링이 강력히 권장됩니다.
임상 팁: 텔미사르탄은 CKD(IRIS 단계 IIa ~ IV)가 있는 고양이에서 6개월 동안 단백뇨 악화를 지연시키는 데 베나제프릴만큼 효과적인 것으로 보입니다.
작용 기전
Telmisartan is a specific Angiotensin II receptor (type AT1) antagonist. By blocking this receptor, it inhibits the detrimental effects of angiotensin II, including:
- Vasoconstriction → lowers systemic blood pressure.
- Aldosterone synthesis → reduces sodium and water retention.
- Renal, vascular, and cardiac remodelling.
In the kidneys, blocking angiotensin II reduces glomerular capillary hypertension and decreases protein in the glomerular filtrate, which helps prevent the triggering or potentiation of interstitial fibrosis.

주의
Blood Pressure Monitoring: Recommended in cats that develop clinical signs referable to hypotension or cats undergoing general anaesthesia. Severe Hypertension: Safety and efficacy for management of systemic hypertension above 200 mmHg has not been investigated. Hepatic Monitoring: Rare increases in ALT activity have been reported, though they typically resolve within a few days of stopping therapy.
