Bronchodilation: 20 mg/kg p.o. q24h or 10 mg/kg p.o. q12h which may be increased to 15 mg/kg p.o. q12h if no side effects on lower dose. Manufacturer only recommends q24h dosing. Some texts indicate q12h dosing of the sustained-release preparation is required to maintain therapeutic serum levels.

개요

테오필린(Theophylline)은 주로 개와 고양이의 소기도 질환 관리에 사용되는 메틸크산틴계 기관지 확장제입니다. 기관지 확장, 점액섬모 청소율 향상, 호흡 중추 자극, PaCO2에 대한 민감도 증가, 횡격막 수축력 증가, 비만 세포 안정화 및 경미한 수축력 증가 효과를 제공합니다. 또한 진경제로 작용하며 가벼운 이뇨 작용을 합니다.

임상 경고: 테오필린은 치료 지수가 낮으므로(치료 혈장 농도 5-20 µg/ml), 독성을 피하기 위해 반드시 제지방 체중(lean body weight)을 기준으로 투여해야 합니다.

작용 기전

Causes inhibition of phosphodiesterase enzymes and antagonism of adenosine and prostaglandin receptors → increased intracellular cAMP → smooth muscle relaxation and bronchodilation. It also alters intracellular calcium and stimulates catecholamine release, providing mild inotropic and diuretic effects.

주의

Administer with caution in patients with:

Severe cardiac disease
Gastric ulcers
Hyperthyroidism
Renal or hepatic disease
Severe hypoxia
Severe hypertension

Note: Most adverse effects are related to the serum level and may be symptomatic of toxic serum concentrations. The severity of these effects may be decreased by the use of modified-release preparations. They are more likely to be seen with more frequent administration.
