항콜린제 / 산동제

트로피카마이드는 산동 및 섬모체근 마비에 사용되는 합성 단기 작용 항콜린제입니다. 작용 발현이 빠르고(20-30분) 작용 시간이 짧아 안구 내 검사를 위한 1차 선택 산동제입니다. > **임상 팁:** 섬모체근 마비 효과보다 산동 효과가 더 뛰어나므로 포도막염과 관련된 섬모체근 경련 완화에는 아트로핀보다 효과가 떨어집니다.

작용 기전: Tropicamide acts as a competitive antagonist of acetylcholine at muscarinic receptors. **Acetylcholine inhibition** at the iris sphincter muscle → relaxation of the sphincter → **mydriasis** (pupil dilation). **Acetylcholine inhibition** at the ciliary body muscles → paralysis of accommodation → **cycloplegia**.

과용량

Systemic toxicity is rare with standard topical ophthalmic use. However, excessive administration or ingestion could theoretically lead to systemic anticholinergic signs such as **tachycardia**, **dry mucous membranes**, **ileus**, and **urinary retention**. Treatment is generally supportive.
