개요
우르소데옥시콜산(UDCA)은 주로 간담도 질환 치료에 사용되는 친수성 담즙산입니다. 독성이 있는 소수성 담즙산으로부터 간세포를 보호하고, 담즙 흐름을 자극하며(이담 작용), 면역 조절 및 항세포사멸 특성을 가지고 있습니다.
임상 팁: 만성 간염, 담관염 및 담낭 슬러지 관리에 매우 효과적입니다.
경고: 완전한 담도 폐쇄가 있는 환자에게는 금기입니다.
작용 기전
UDCA is a hydrophilic bile acid that inhibits ileal absorption of toxic, hydrophobic bile acids → reduces their concentration in the systemic bile acid pool. Hydrophobic bile acids are toxic to hepatobiliary cell membranes and potentiate cholestasis. UDCA also stimulates biliary secretion (choleretic effect), protects hepatocyte membranes from detergent effects, and exerts immunomodulatory and anti-apoptotic effects on liver cells.

- Severe acute hepatitis
- Cholecystitis
부작용
- Mild gastrointestinal upset (diarrhea, vomiting)
- Hepatotoxicity (rare, typically only if administered during biliary obstruction)
주의
Do not use in patients with complete extrahepatic biliary obstruction.
Use with caution in patients with complications of advanced liver disease (e.g., ascites, hepatic encephalopathy).

Well absorbed from the small intestine (primarily the ileum). Absorption is enhanced when administered with food.
분포
Extracted by the liver from portal blood and enters the enterohepatic circulation. Very little reaches systemic circulation.
대사
Conjugated in the liver with glycine or taurine, then secreted into bile.
배설
Excreted primarily in feces. Unabsorbed drug is degraded by colonic bacteria.
과다투여
Acute overdosage is unlikely to cause severe toxicity but may result in diarrhea. Treatment is symptomatic and supportive.
